New Patient Registration

The doctors welcome new patients who live within our practice area.  As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a consultation with the doctor. 

Medical treatment is only available from when the registration process has been completed in full.  Please contact reception for further information.

Registration Pack Documents

All parts of the documents below must be printed off then filled in to take with you to the practice on your first visit.

All new patients should attend Heathville Medical with their completed packs so that we can verify ID documents. This is not a facility available at our branch site of 38 Warwick Avenue. You will not be registered with the practice until you have attended the surgeries with your ID etc and had a new patient health check .
